Position Title:  Case Manager


Location: Warren, Mi and Rose City, MI


Department: Clinical Services


Payroll Status:  Contractual




The Safehaus Case Manager shall possess a minimum of a Bachelor’s Degree in Psychology or Social Work and have at least (1) year of experience working with children and adolescents in a residential or hospital setting.  The Case Manager shall have the following duties:


  1. Conduct child/ adolescent assessments.

  2. Set up aftercare appointments for residents and their families.

  3. Coordinate all plans of service, medication, and therapy appointments.

  4. Coordinate weekly Clinical Team Meeting.

  5. Chart Reviews

  6. Case Coordination with Funding agency.

  7. Coordinate care with the hospital liaison.
